   As of June 3^rd 2019, N.C. General Statute 90-113.70 known as the “Stop Act”, requires us to report certain prescriptions to the NC Controlled Substance Reporting System.  Due to this reporting, we will need to be sure we have the following information:

 Date of Birth of pet owner (That’s right! Your birthday, not your pet’s):

 You are probably aware of having to do this at your own pharmacies.
